Evaluations

A Cognitive Assessment is a limited evaluation that assesses verbal and nonverbal abilities, providing a Full-Scale Intelligence Quotient (IQ score). This assessment is designed for individuals who are seeking to identify areas of cognitive strength and weakness.
Our Capacity/Competency Evaluation determines an individual's ability to provide consent for treatment and release of medical records. This evaluation includes a clinical interview and a Capacity Evaluation Questionnaire.
An ID/DD Determination Evaluation is designed to determine if an individual has an intellectual or developmental disability. This evaluation includes a diagnostic interview and the administration/scoring/interpretation of testing that determines adaptive behavior functioning and cognitive functioning abilities.

Smith Psychological Services offers training and supervision for graduate-level students pursuing a degree in psychology.
